Please give details of referees from your two latest periods of employment. These should be your Line Manager, your Senior or HR Department. Please give as much detail as possible, as a delay in obtaining references could delay your start date if successful. We implore you to inform them of the need to expect a reference from Mindset.

If you have recently left full time education or have only had one manager, please give the name of someone who can give a character reference. NOTE: We are not able to take up references from friends or family.

If you have held any job in care, we are obliged under Care Quality Commission regulations to obtain a satisfactory reference from your most recent care employer (this may have been a few years ago) If this applies to you please list the referee for your last care role on the second page.
